**Medical records ARE required! Records can be mailed in, faxed in or brought to our office but MUST BE IN OUR OFFICE BY MARCH 15TH for this particular clinic**
IMPORTANT: You must have a Montana State issued ID or Montana State issued drivers license. Medical marijuana cards from out of state or out of state drivers licenses/IDs are not allowed to be used for the Montana program

& Education Center in Ennis
IMPORTANT: medical records ARE required for these clinics. You should give yourself plenty of time to obtain copies of your records from your primary care physician. It is not necessary to obtain copies of your entire file, patients need only the pages that pertain to their "debilitating medical condition", as outlined in Montana Initiative 148. The records need to show a diagnosis by your primary care physician and recent doctors notes showing any and all prior treatments for your ailment.
This service is offered as a physician consultation only. The reviewing doctor would not be your primary care physician. The doctor consultation will consist of a review by the doctor of your past medical history and an interview with the doctor to discuss your medical history with you. If it is determined by the doctor that your debilitating condition is acceptable as per the Montana Medical Marijuana Act definition of debilitating condition they will sign your recommendation for medical marijuana that you must send into the state with your New Patient Application to receive your Montana Medical Marijuana Program patient card. Patients that are deemed ineligible, will not have to pay for their appointment.
